
Methylation Issues and Persistent Symptoms
You can eat clean, take supplements, sleep eight hours, and still wake up exhausted with brain fog, anxiety, headaches, or stubborn weight gain. When standard lab work comes back “normal,” it is easy to assume the symptoms are in your head. They are not. Methylation issues may be one piece of the bigger picture affecting how your body creates energy, manages inflammation, processes hormones, and responds to stress.
Methylation is often reduced to a conversation about an MTHFR gene variant and a methylated B vitamin. That oversimplification leads people to self-treat with high-dose supplements, feel worse, and conclude that functional medicine does not work. The truth is more nuanced: methylation is a body-wide process, and the right next step depends on your symptoms, genetics, nutrient status, medications, gut health, toxic burden, and lifestyle.
What Are Methylation Issues?

If your provider only checks a basic blood count and a standard B12 level, important details can be missed. A serum B12 result alone does not always show whether B12 is reaching cells or whether it is being used effectively. Likewise, a “normal” lab range does not always explain why someone is symptomatic.
A thorough evaluation may consider your full health history alongside targeted testing. Depending on your case, that can include folate and B12 status, homocysteine, methylmalonic acid, iron markers, thyroid markers, vitamin D, blood sugar and insulin patterns, liver function, inflammation markers, and nutrient testing. Genetic information can be useful in context, but it should not replace clinical investigation.
Your provider should also ask questions that are frequently skipped: Are you taking acid-suppressing medication, metformin, or hormonal birth control? Have you had bariatric surgery, chronic diarrhea, reflux, constipation, or food sensitivities? Do you have a history of restrictive dieting, heavy alcohol use, high stress, mold exposure, or frequent antibiotics? Each answer can change the plan.

Gut health cannot be ignored. Chronic inflammation, low stomach acid, dysbiosis, celiac disease, inflammatory bowel disease, and poor digestive capacity can all interfere with nutrient absorption. They can interact with medications, complicate certain health conditions, or worsen symptoms when used without proper assessment.
This is especially relevant for people with a history of anxiety, insomnia, migraine, bipolar disorder, histamine-related symptoms, or medication use. Some people do better with lower doses, different nutrient forms, or a plan focused first on digestion, iron, thyroid function, or blood sugar. The correct approach depends on the person in front of you.
When Persistent Symptoms Deserve a Deeper Investigation
If you are tired of being told your labs are fine while your body is clearly telling you otherwise, do not settle for vague reassurance.
